It really depends on what state you live. A form that works in one state may not work in another. Plus, some states do not have legal separation. The best thing is to do a search like "legal separation forms, Your State Here", be sure to substitute "Your State Here" with your actual state! Also, you can try the search with or without the quotation marks.
